Contributing to the Rise in Cases: Heavier Children, Lower Cribs, Older New Mothers, and Frequent Smartphone Scrolling.

Tara Heglar was picking up her son from his crib when the pain started, a searing sharpness on the side of her right hand.

It began when Charlie was about a month old and continued to worsen over the next half year. The snap of pain would return when she used her iPhone, lifted her son or even cradled his head while he nursed. "It felt like I had bruised my bone," says the 38-year-old Ms. Heglar, a business analyst in Concord, N.C.

Orthopedic surgeons estimate that between one-quarter and one-half of new mothers experience symptoms of De Quervain's. When common pain relievers don't ease the inflammation, patients are getting steroid injections, splints and even surgery. Ms. Heglar says she wore a splint after getting a cortisone shot and hasn't had problems since. An orthopedic surgeon diagnosed Ms. Heglar with mommy thumb, formally known as De Quervain's tendinitis, an inflammation of the tendons below the thumb down to the wrist. Although the condition has been around as long as parents have hoisted their children, doctors say a combination of heavier children, older new mothers, cribs that are lower to the floor and frequent scrolling and typing on smartphones has sent the number of cases skyward.

People with De Quervain's typically experience pain on the thumb side of the wrist that radiates up the forearm. Sometimes the symptoms start during pregnancy. Doctors aren't sure why, but they believe that swelling adds pressure to already stressed tendons, exacerbating the irritation. For many women (and some men), it gets worse when they are constantly lifting a child, bending down into the crib, putting their hands in an "L" shape under the child's armpits and lifting so that much of the weight is on the thumbs.

In a study published last year that looked at more than 11,000 cases of De Quervain's culled from a large data base of military personnel, researchers at the University of Colorado School of Medicine found that the condition occurred more than four times as frequently in women as in men. The study also found that people over 40 years old were more than three times as likely to get De Quervain's as younger people.

Rachel Rohde, an orthopedic surgeon in Royal Oak, Mich., says most of her patients with De Quervain's are mothers, but she also sees some fathers, grandparents and nannies with the condition. As the mother of a newborn herself, Dr. Rohde said she's careful to put a pillow under the baby's head while nursing so the weight of the baby is not resting solely on her hand and straining her thumb. She also says she tries to use a "scooping" technique to lift the baby out of the crib. By placing the weight in the palms or on the forearms, it avoids putting the thumb out of alignment to the rest of the hand and stressing the tendons, she says.

Still, Dr. Rohde says, the automatic motion for picking up a baby is usually to lift from under the baby's arms, which places the strain on the thumb. "Sometimes you can't avoid it," she says.

Children are getting heavier, on average. In 2004, 10.1% of children aged 6 months to 2 years were overweight, compared with 5.7% in 1980, according to the most recent data from the Centers for Disease Control and Prevention.

Katy Lawson, 29, of Lantana, Texas, says she never had problems with her hands until her son was about a month old. She began noticing sharp pains down her hand when she tried to lift him or grip something, and a bump was forming on her wrist. Although Ms. Lawson's son is not particularly heavy, she says he often sleeps in a portable playpen that is low to the ground. Many cribs also are now lower to the floor for safety reasons, and doctors say that creates more stress on the hand when lifting a baby.

Two months later, Ms. Lawson says she went to see a hand surgeon. When he asked if there was a new baby in the house, she said, "It's funny that you mention that." She says she has received two cortisone shots on the wrist, beneath her thumb. She was also given a splint to wear for a few weeks.

The treatment hasn't helped, and Ms. Lawson says she is scheduled for surgery this month. She says her thumb doesn't hurt as much when she picks up her son, because she is careful about she lifts him. "It just hurts randomly. Sometimes when I'm just lying in bed it catches," she says.

Mommy thumb mainly affects two tendons—the abductor pollicis longus, or long thumb abductor, and the extensor pollicis brevis, or short thumb extensor. The tendons come into play when the thumb is moved in a different plane from the hand, as when placing hands flat on a table and lifting the thumb toward the ceiling.

If ice and anti-inflammatory drugs don't help, most patients are fitted with a splint that prevents them from moving the thumb and given a steroid injection, which decreases the inflammation, says Stephen Troum, a hand surgeon in Bedford, Texas, and a spokesman for the American Society for Surgery of the Hand, a doctors' association.

Dr. Troum, who treated Ms. Lawson, himself had hand surgery for De Quervain's when he was a resident after aggravating his wrist doing a lot of surgery. He says that if patients don't feel relief after one or two steroid shots, he usually recommends surgery. Too many injections can weaken the tendons to the point of rupture, he says.

Michelle G. Carlson, a hand surgeon at the Hospital for Special Surgery in New York City, developed De Quervain's herself after giving birth. "I thought I was really cautious, but when I had my first child, I started feeling sore and I made myself a splint right away," she says.

She says mommy thumb is the main reason she sees De Quervain's patients, although others can develop it from tasks that repeatedly stress the tendons around the thumb. "A lot of people will say, 'Oh my child is so heavy,' " Dr. Carlson says. Heavier children tend to start walking later so mothers are doing a lot more lifting for a longer time. "You don't see this develop when the child is 3 and they're walking on their own," she says.

Shopping @ LittleWhiz

Ok la kan, segelintir barang-barang bayi memang mahal. Aku beli 5 barang dah melebihi RM 300, dan beratnya baru 2 kg.  Jadi dapatla penghantaran percuma.

Pilihan Pelbagai Mod Bayaran
Masa aku beli ni gaji tak masuk lagi. Tapi LittleWhiz terima bayaran melalui kad kredit, jadi selesai la masalah.. Selain kad kredit, bayaran melalui bank secara atas talian pun diterima. Proses pembayaran pun selamat, mudah dan pantas..

Maklumbalas Pantas

Little Bean Sterilizer & Warmer.. 2. Little Bean Infra Red Ear Thermometer.. 3. Bumble Bee Stroller Pad.. 4. Bumble Bee Fitted Crib Sheet.. 5. Bumble Bee Swaddle Blanket..
Lagipun, aku nak kumpul mata ganjaran hehe..

Sekarang Baru Ku Tahu Rasanya..

Sekarang baru ku tahu rasanya disengat... kala jengking (KJ)! Ikutilah kronologi di bawah ini.

Sabtu, 7hb Nov. 209

8.30 mlm
Aku cuci muka sebab nak bersiap2 keluar lepak2 kat dataran. Napi pun nak tengok final Piala Malaysia..

8.35 mlm
Aku capai tuala muka kat belakang pintu nak lap muka. Tengah2 lap muka terasa tangan pedih macam kena selumbar/kaca/jarum.. Dengan kehairanan aku balik2kan tuala.. Kemudian aku campak tuala tu kat lantai seraya menjerit.. terkejut seh.. Seekor KJ sebesar jari kelingking dengan ekor yang menegak tengah port baik nye kat belakang tuala muka aku.. Aku menangis sebab terkejut+bimbang.. 1st time tengok KJ live, terfikir pasal bisa, pasal baby lagi.. Napi genggam pergelangan tangan aku, capai tudung dan terus bawak aku pergi rumah Kak Lela.. dia biar je tuala tu..

8.37 mlm
Kebetulan Kak Lela baru balik, aku duduk depan rumah dengan Kak Lela sekeluarga. Minta tolong Kak Lela sarungkan tudung. Dalam blur2 tu terpikir gak nape Napi suruh gi rumah Kak Lela? Napi suruh aku genggam pergelangan tangan, pastu dia masuk rumah balik diikuti oleh Arip, anak teruna Kak Lela..

8.40 mlm
Arip keluar rumah aku sambil berkata, "dah abis dah, terbelah dua!". Napi keluar rumah dengan muka kool je.. cam mafia lak.. Dia kunci rumah, bukakan pintu kereta untuk aku, kemudian dia pinkan kerongsang kasi molek tudung (baru nak suruh dukung masuk kereta, tapi dah 60 kilo ni rasanya terlebih sudah huhu).. Kami ke poliklinik Nge kat Pt. Raja.

9.00 mlm
Singgah Petronas beli air mineral nak makan ubat. Pastu kami tetap gi BP.. aku trauma jap nak tengok rumah tu.. terbayang2 lagi KJ dengan ekornya yang tegak tu.. Akhirnya sampai bandar aku dan Napi tengok Piala Malaysia kat bus stand lama sambil pekena tempe goreng.. Habis Piala Malaysia baru tapau makanan. Dataran penuh sangat, ada expo ape tah..

Setakat hari ni belum ada reaksi alahan lagi, mak pun kata dah ok.. cuma masa nak baik nanti mungkin kawasan kena sengat tu membengkak dan gatal2.. Nasib baik tangan.. kalau muka? huhu... trauma nye rasa.. tuala muka tu aku suruh Napi buang, taknak tengok lagi dah..

Napi kata harus bunuh sebab haiwan berbisa.. Tapi biasanya Napi memang sayang binatang.. Kumbang, lipas ngan cicak kalau dia dapat tangkap hidup2 buang kat luar.. Kalau aku dapat, dah lama kena spray Mortein Knocdown.. Kalau tangkap lipan pun dia cuma buang sengat pastu campak kat parit.. Sayang Napi..

What is my baby doing...?

Dah masuk bulan ke-5 ni dah gagah sket la.. dah berani nak jalan2 sana-sini & lasak2 sikit. Dulu aku takut nak drive, sebab aku ni gelabah.. takut terkejut2 je.. skang dah ok dah.. siap drive malam aritu.. adik aku lak yang takut tengok hu..

Closer to birth, the fetus sleeps 85 or 90% of the time: the same as a newborn.
Abis tu rajin jek gerak? Tu tengah tidur ke atau sedar?
Awake or asleep, the human fetus moves 50 times or more each hour, flexing and extending its body, moving its head, face, and limbs and exploring its warm, wet compartment by touch. Heidelise Als, Ph.D., a developmental psychologist at Harvard Medical School, is fascinated by the amount of tactile stimulation a fetus gives itself. “It touches a hand to the face, one hand to the other hand, clasps its feet, touches its foot to its leg, its hand to its umbilical cord,” she reports.
Along with such common movements, DiPietro has also noted some odder fetal activities, including “licking the uterine wall and literally walking around the womb by pushing off with its feet."
Fetuses react sharply to their mother’s actions. “When we’re watching the fetus on ultrasound and the mother starts to laugh, we can see the fetus, floating upside down in the womb, bounce up and down on its head, bum-bum-bum, like it’s bouncing on a trampoline,” says DiPietro. “When mothers watch this on the screen, they laugh harder, and the fetus goes up and down even faster. We’ve wondered whether this is why people grow up liking roller coasters."
Fifer has found that fetal heart rate slows when the mother is speaking, suggesting that the fetus not only hears and recognizes the sound, but is calmed by it.
DeCasper and others have uncovered more mental feats. Newborns can not only distinguish their mother from a stranger speaking, but would rather hear Mom’s voice, especially the way it sounds filtered through amniotic fluid rather than through air. They’re xenophobes, too: they prefer to hear Mom speaking in her native language than to hear her or someone else speaking in a foreign tongue.

Buku oh Buku..

Aku baru beli 3 buku ni.. Kalo ikutkan kat library UTHM ada, Napi leh pinjam.. Tapi sebab aku memang minat buku, saja nak tambah koleksi buku kat rumah..

Dr. Miriam Stoppard's New Pregnancy and Birth Book

Aku beli buku ni pas puas mencari buku kehamilan yang terbaik kat
Amazon. Kalau nak baca tu kena banyak mengucap sikit, sebab grafiknye banyak 18++!!! Tapi memang best la, lebih2 lagi bila kita dapat mengikuti perkembangan bayi dan perubahan badan kita setiap bulan..

Amalan Ibu Mengandung

Buku ni aku beli kat kedai buku berdekatan dengan UTHM kampus bandar.. Sebab aku kena marah ngan abang aku aritu.. Abang aku bising sebab aku pandai2 surf Internet cari amalan ibu mengandung waima tak tau sumber dari mana. Untuk menyedapkan hati abang aku, buku ni aku wat
backup he.. Sekurang2nye amalan Ustazah Siti Nor Bahyah.. Taklah susah sangat, amalannya mudah dilakukan.. Rege RM 19.90. Puas ati ka Along???

Panduan dan Koleksi Nama-Nama Mulia Dalam Islam

Sebenarnya sebelum ni aku pernah beli buku seumpama ni.. Saja gatal beli lagi sebab
cover buku nye sangat comel. Isinye biasa je. Tak baper lengkap, tapi boleh la.. Rege RM12.00.

Folic acid, sometimes called folate, is a B vitamin (B9) found mostly in leafy green vegetables like kale and spinach, orange juice, and enriched grains. Repeated studies have shown that women who get 400 micrograms (0.4 milligrams) daily prior to conception and during early pregnancy reduce the risk that their baby will be born with a serious neural tube defect (a birth defect involving incomplete development of the brain and spinal cord) by up to 70%.

Kat Wikipedia pula, begini maklumatnya:

Folic acid is an important nutrient for women who may become pregnant. Adequate folate intake during the periconceptional period, the time right before and just after a woman becomes pregnant, helps protect against a number of congenital malformations including neural tube defects (which are the most notable birth defects that occur from folate deficiency).[18] Neural tube defects (NTDs) result in malformations of the spine (spina bifida), skull, and brain (anencephaly). The risk of neural tube defects is significantly reduced when supplemental folic acid is consumed in addition to a healthy diet prior to and during the first month following conception.[19][20] The protective effect of folate during pregnancy goes beyond NTDs. Supplementation with folic acid has been shown to reduce the risk of congenital heart defects, cleft palate, limb defects, and urinary tract anomalies [21] Women who could become pregnant are advised to eat foods fortified with folic acid or take supplements in addition to eating folate-rich foods to reduce the risk of some serious birth defects. Taking 400 micrograms of synthetic folic acid daily from fortified foods and/or supplements has been suggested. The RDA for folate equivalents for pregnant women is 600-800 micrograms, twice the normal RDA of 400 micrograms for women who are not pregnant[22]
Recent research has shown that it is also very important for men who are planning on fathering children, reducing chromosomal defects in their sperm.[23]

Pendekkan cerita, asid folik ni supplement penting untuk bakal ibu. Seelok-eloknya ambil 3 bulan sebelum mengandung, dan diteruskan semasa dah mengandung.

Jadi semalam aku pergi la beli kat farmasi. Akak farmasi tu cakap, 1 papan RM1. Kalau kat Wikipedia, katanya begini:

The risk of toxicity from folic acid is low.[79] The Institute of Medicine has established a tolerable upper intake level (UL) for folate of 1 mg for adult men and women, and a UL of 800 µg for pregnant and lactating (breast-feeding) women less than 18 years of age. Supplemental folic acid should not exceed the UL to prevent folic acid from masking symptoms of vitamin B12 deficiency.[80]
Research suggests high levels of folic acid can interfere with some antimalarial[81] treatments.
A 10,000-patient study at Tufts University in 2007 concluded that excess folic acid worsens the effects of B12 deficiency and in fact may affect the absorption of B12. [82]

Nampak macam takde kesan jangka panjang.. Untuk mengesahkan maklumat ni, aku call mak aku, bekas bidan HSA. Memula segan nak cakap, tapi terpaksa gak la tanya.. Dah kena marah sebab tak jumpa doktor dulu sebelum beli (ingat ya pesanan penting ini dalam membeli supplement:  DAPATKAN NASIHAT DOKTOR TERLEBIH DAHULU). Tapi yang lucunye mak aku suruh tanya Aai sebab bakal pharmacist mesti tau (bukan bidan lagi tau ek?). Aku pun call Aai lak tanya..

Aku pula jenis pemilih kalau bab sayur-sayuran ni. Lepas tu nak pastikan pengambilan seharian tu mencukupi, jadi terpaksa la aku ambik asid folik ni hari-hari.. Lagipun ada pendapat mengatakan asid folik ni boleh meningkatkan kesuburan, baik lelaki mahupun perempuan.. Hmm.. Rasa-rasa betul ke..
